Status Collapse: One Error in Five Fields
Approved, queued, funded, reopened and active all hide several states in one word. The fix is the same in every field.
Across unrelated fields the same mistake keeps appearing. A rich chain of separate facts gets compressed into one status word, and decisions are then made on the word.
In medicine, approved can hide the formulation, the label version, later safety communications and a separate coverage decision. On the grid, queued hides whether a project has an agreement, and history shows most requested capacity does not reach operation. In water infrastructure, a national count hides known lines, unknown materials and estimates, and funded hides whether anything was spent or replaced. In aviation, an authority returning hides the status of each recommendation.
This is the pattern. The noise is the feeling that each case is unique to its industry. The vocabulary, the regulators and the risks do differ, which is why every domain needs its own sources and its own qualified review. The structural error does not differ.
The repair is plain. Separate the states into their own fields. Give each one a source, a date and an owner. Keep superseded values visible. Show what is unknown. That discipline transfers between fields. Authority in a field does not.
Limit. These are systems observations from public records. They are not medical, engineering, investment or legal conclusions, and none of the cited cases has had outside expert review by Delta Arc.
Sources
One word hides several states
Approved, active, poor, final, in force. A single status word stands in for separate facts, each with its own source and date.
Approved Is Not One State
Public evidence states for one class of treatment. Research only.
What a Poor Bridge Rating Can Establish
A National Bridge Inventory Poor rating records component deterioration; it is not by itself a finding that a bridge is unsafe.
What FMCSA Carrier Records Can and Cannot Establish
Operating authority, insurance filings, safety ratings and SMS data are separate records; active authority is not a safety rating.
